Wondering How You Can Complete Your Assessment?
Follow the instructions below for the plan you are enrolled in. If you are not certain which plan you are in, click here to go to the Washington State Health Care Authority website to verify your PEBB coverage.
Q: Who is eligible?
A: All benefits-eligible CBC employees who are enrolled in a PEBB medical plan (Uniform Medical Plan or Group Health) are eligible for the drawing. Since the health assessment is only taken once per year, employees who completed their health assessment as part of the competition earlier this year will also be entered into the drawing.
Q: How do I enter the Kindle drawing?
A: Eligible members who submit proof that their health assessment was completed by December 31, 2013 will be entered into the random-prize drawing. Proof of completion can be a printout or screenshot including your name and date of completion of the health assessment and can be submitted in hard copy to the Human Resources Office or by e-mail to firstname.lastname@example.org. Proof of completion must be submitted by 4:30pm on December 31, 2013. Entries submitted after that point will not be eligible for the drawing. If there is any confidential health information included in your printout, please be sure to black out all the information prior to submitting to the Human Resources Office.
Q: What if I already completed my health assessment earlier this year?
A: Health assessments completed between January 1, 2013 and December 31, 2013 are eligible for entry into the random-prize drawing. The same entry rules apply in terms of proof of completion with name and date of completion of the heatlh assessment. We are not privy to reports or other information from the different companies, so a printout or a screenshot is needed for a drawing.
Q: When will the drawing take place?
A: The drawing will take place on Monday, January 6, 2013.
A: No, if you are the random-prize drawing winner, you will be contacted by e-mail or via other contact information on file.
Q: Will there be any cost to me if I win the drawing for the Kindle?
A: No and yes. The Kindle Fire was donated by one of our own CBC employees who won the prize at a conference attended as a CBC employee. You will be responsible for adding any bells and whistles if desired.
Q: Who is eligible to take the Health Assessment?
A: All benefits-eligible CBC employees and their dependents (must be 18 years of age) who are enrolled in a PEBB health plan are able to take a health assessment. Only benefits-eligible CBC employees who are enrolled in PEBB coverage and complete their health assessment are eligible for the random-prize drawing.
Q: Who will have access to my health assessment information?
A: Your health assessment information is completely private and confidential and remains with your health plan. You choose who you share the information with. Similar to your other health information, your health plan must keep your information personal as required by the Health Insurance Portability and Accountability Act (HIPAA) of 1996.
Q: What will be done with my answers to the health assessment?
A: As an employer, the State will only see a broad picture of employees' health as a group. Additionally, by reaching a certain completion rate, the College will receive an aggregate report with an overall snapshot of information for all employees, but nothing personal linking you to your answers. This means individuals remain anonymous. This snapshot will give the College information about better ways to focus our wellness initiatives, and help Washington Wellness coordinators and staff identify major health risks in the population, such as lack of physical activity.
Washington Wellness works with agencies and institutions of higher education to provide online and worksite programs and tools to assist employees who choose to reduce their health risks and improve their health. No staff at the College or Washington Wellness will see individual-level information.
Q: Does the State or my manager know if I completed the health assessment?
A: No, not unless you tell your manager. Only you and your health plan know if you completed the health assessment. Washington Wellness tracks the number, but not the identity, of employees who complete the health assessment.
Q: Will the health assessment affect my insurance coverage, premiums, or share of health care costs?
A: No, the health assessment is not used to guide insurance coverage decisions or to affect your health care premiums or costs.
Q: How long will it take me to complete the health assessment?
A: It will take you approximately 20-30 minutes to complete the health assessment, which can be completed on work time. Please check with your supervisor about arranging this. You will need to allow extra time to set up a username and password if it is your first time completing the health assessment, or if you are new to the system. If you are not able to log in to your account immediately, it could take between 2-5 days for your access code to be mailed to you.
Q: What kinds of questions are on the health assessment? Do I have to answer them all?
A: The health assessment includes questions regarding physical activity, food selection, tobacco and alcohol use, women's health, chronic conditions, preventative services and health screenings and a few others. You may skip questions you are not comfortable answering.
Q: Is there an incentive for taking the health assessment?
A: The health assessment provides you with valueable feedback regarding your personal health. It gives you knowledge about how choices you make every day can affect your health, and also equips you with the tools necessary to make a plan on your own, or with the help of your healthcare provider, to address your risks and make choices to improve your health!
