High School Academy students need the following to obtain a Student ID (SID) card:
- HSA College Placement/Assessment Authorization Voucher
- Student ID number (on form)
- Picture ID (ie: Driver's License)
HSA acceptance will be verified each quarter to receive a new quarterly sticker. Quarterly stickers may be obtained from the High School Academy office after the first quarter.
Only one SID card will be issued per student. A fee of $3.25 will be charged for replacement of the card. The fee must be paid at Hawk Central prior to receiving a new SID card.
Appropriate picture ID:
- Driver's License
- United States Passport
- Armed Forces ID
- Employee photo ID
